Three kinds of calls
Families and general inquiries
Questions about hospice, eligibility, or what care would look like for someone you love.
Speak with a NurseProfessional referrals
For physicians, discharge planners, case managers, and facility staff coordinating a referral.
Referral guidancePatients already in our care
Current patients and families should use the number provided at admission to reach the clinician on call.
What to expect after admissionWhat happens when you call
- 01
Someone picks up. Our team is available 24 hours a day, 7 days a week. Tell us who you are calling about and what has changed.
- 02
We ask about the diagnosis, the recent trajectory, and who their doctor is.
- 03
We talk through whether hospice may be appropriate and what an evaluation involves, and our team can help confirm availability for your location and care needs.
- 04
If hospice is not the right answer right now, we say that too, and tell you what might help instead.
There is no cost to the call and no obligation to begin care.
Service base
Arcadia, California. We serve all of Los Angeles County and surrounding areas, and we travel to patients rather than asking them to come to an office. See areas we serve.
If this is urgent
If someone is in immediate danger, call 911. For anything else, call the number above. Our team is available 24 hours a day, 7 days a week and will tell you what to do next. Patients already admitted should use the number given at admission.
Language
Tell us which language you would prefer to use, and we will confirm what we can arrange before anything is scheduled.
Accessibility
If you need information in another format, or help reaching us by phone, tell us and we will work it out. If part of this website is hard to use with a keyboard, a screen reader, or enlarged text, tell us and we will fix it.
Privacy and medical information
The home page has a short intake form for families and professionals who would rather write than call. It asks for contact details, the city or ZIP where care is needed, and a brief note about the situation: nothing more.
Please do not put medical record numbers, a full medical history, or other protected health information into that form; a nurse will call you and arrange a secure way to receive records. We do not run advertising trackers on these pages.
Clinicians sending records should call intake first and use the secure method we provide. Do not email protected health information to a general inbox.
If this is an emergency
Call 911. For a patient already on hospice, also call the hospice on-call nurse after emergency services are on the way. If you are in emotional crisis, call or text 988 for the Suicide and Crisis Lifeline.
No forms, no funnel: just a phone call
Whether you are gathering information or need help today, calling is the fastest way to get a real answer.
