Privacy

Hospice of Grace & Palliative Care, Inc. ("Hospice of Grace," "we," "us") publishes this page about this website. It is not the Notice of Privacy Practices that governs your care record; that notice is provided to patients and families at admission.

Information this website collects

This website is an information site. It has no patient portal, no chat widget, and no account to create. You cannot upload documents or request records through these pages, and we do not ask for a diagnosis, a medical record number, or an insurance number. The one place you can send us information is the short intake form on the home page, described below.

As built, this site does not include advertising pixels or analytics scripts, and we do not build advertising profiles of visitors. Our hosting provider processes the standard technical information required to serve a page and protect the site from abuse: the page requested, the time, browser type, and an approximate location derived from an IP address. Fonts are loaded from Google Fonts, which receives the request from your browser as part of serving the file.

If we add analytics or advertising tools later, this page will say so before or when they go live, and the cookie section below will list them.

Information you choose to give us

If you call or email, we receive what you choose to share, typically a name, a callback number, and something about the situation. We use it to answer you and, if you become a patient, to provide care.

If you send the home page intake form, we receive the name, phone number, email address, city or ZIP, and the note you write, plus the answers you select about where care would happen and how soon you need to talk. Those details are stored in our intake system and shared with the clinicians who would arrange an evaluation. We do not use them for marketing.

Please do not send medical details through this website, and do not send protected health information to a general email address. Ask first, and we will tell you how to send clinical information securely.

Health information is governed separately

Information collected in the course of treatment is not governed by this page. It is governed by our Notice of Privacy Practices, which is provided at admission, and by federal and California law. If you need a copy of that notice, ask us and we will provide one.

How information is shared

We do not sell personal information. Information may be shared with service providers who host or support this website under confidentiality obligations, when required by law or legal process, to protect the safety and rights of patients and the public, or in connection with a merger or transfer of assets. The full description of how health information may be used and disclosed is in the Notice of Privacy Practices described above.

Your California privacy rights

If you are a California resident, the California Consumer Privacy Act, as amended by the California Privacy Rights Act, gives you the right to know what personal information we hold about you, to request a copy of it, to ask us to correct inaccurate information, to ask us to delete it, and to be treated without discrimination for exercising those rights. We do not sell personal information.

To make a request, email info@hospiceofgrace.com or call (818) 351-5250. We will verify your identity before acting on a request, and we will tell you if an exception applies.

Under California Civil Code Section 1798.83, California residents may also ask about disclosure of personal information to third parties for their direct marketing purposes. We do not disclose personal information for that purpose.

Cookies

This website does not set advertising or analytics cookies of its own. Your browser may store a small amount of technical data needed to load pages reliably, and browser settings can clear or block it. If we introduce cookies that track visitors, we will describe them here and ask for consent where the law requires it.

Security and retention

We use reasonable administrative, technical, and physical safeguards for the information we hold. No method of transmission over the internet is completely secure, and we cannot promise absolute security. Website inquiries are kept only as long as needed to respond and to meet legal obligations.

Children, and links to other sites

This site is written for adults making care decisions and is not directed at children under 13. Pages link to outside organizations such as Medicare.gov, the National Institute on Aging, and the CDC; their privacy practices are their own and we do not control what they collect.
